Urban Model Initiative is designed to serve individuals who have been diagnosed with mental health and substance abuse disorders. The program utilizes innovative treatment modalities and targets participants who are considered "high utilizers" of services.  People-centered, recovery based services are provided in conjunction with area behavioral health providers.  Transitional, supportive and permanent supportive housing opportunities are available through this program.
