POSITION TITLE: Case Manager

RESPONSIBILITIES/DUTIES:
Full-time position, Wednesday – Sunday 4PM to 12AM. Assess, record and monitor client progress with finances, employment, nutrition, basic needs, housing, physical health, mental health and substance abuse related issues. Serve as advocate for clients in obtaining services and link individuals to community resources.
QUALIFICATIONS:
Bachelor’s degree in the behavioral sciences and at least 2 years experience in counseling. and CAC preferred; or equivalent combination of education and experience. Co-occurring competencies and credentials are a plus.

POSITION TITLE: Counselor

RESPONSIBILITIES/DUTIES:
Full-time position, three days a week 6AM to 2PM and two days 11AM to 7PM. Conduct screening and intake, orientation and multi-dimensional assessment of clients. Assist clients with substance abuse disorders and co-occurring issues using approved methods of counseling, case management and crisis intervention. Prepare and maintain records on assigned caseload. Understanding of and ability to provide integrated care for individuals with co-occurring disorders.
QUALIFICATIONS:
Bachelor’s degree in the behavioral sciences and at least 2 years experience in counseling. MATS certified and/or CAC and SCCD preferred.

Ability to establish and maintain professional relationships with clients, colleagues and supervisors.
QUALIFICATIONS:
Minimum of an Associate’s degree in behavioral health and at least 2 years experience with substance abuse/mental health.

Full-time position: Monday – Friday.
Sweep, mop, scrub and vacuum hallways, stairs, office and client spaces. Ensure that exterior of building and grounds are kept clean. Inventories janitorial supplies and reorders as appropriate. Assists maintenance crew and performs miscellaneous errands as required.
QUALIFICATIONS:
High school diploma or general education degree (GED). Some custodial experience and/or training preferred. Valid CT driver’s license may be required. Ability to function independently. Considerable knowledge of federal and state laws and regulations regarding client confidentiality. Ability to project a professional image and establish professional relationships. Excellent communication skills.
